He claims that overdiagnosis “is the biggest problem posed by modern medicine,” and backs that assertion up with a barrage of facts, charts, and graphs. This is information, he says, that is downplayed or simply ignored by individuals and groups promoting the notion that earlier diagnosiswhether for prostate cancer or diabetestranslates to better health. Indeed, Welch says, just the converse is more often true. In an overwhelming number of circumstances, early diagnosis turns healthy, asymptomatic people into patients who require a variety of medical interventions with no benefit, even exposing them to unnecessary harm. Worse, overdiagnosis can render perfectly healthy people uninsurable. Furthermore, instead of lowering health-care costs, all those scans, screenings, and tests actually raise costs by overtreating people who will never benefit from said treatment. His point is that both physicians and patients need to be skeptical and understand all the data (pro and con) surrounding prescreening for possible illness. Conventional wisdom is that more diagnosis, especially early diagnosis, means better medical care. Reality, says Dr. Gilbert Welch – author of "Overdiagnosed," is that more diagnosis leads to excessive treatment that can harm patients, make healthy people feel less so and even cause depression, and add to escalating health care costs. In fact, physician Welch believes overdiagnosis is the biggest problem for modern medicine, and relevant to almost all medical conditions. Welch devotes most of his book to documenting his concerns via examples of early diagnosis efforts for hypertension, prostate cancer, breast cancer, etc. that caused patient problems.
Welch provides readers with four important and generalizable points. The first is that, while target guidelines are set by panels of experts, those experts bring with them biases and sometimes even monetary incentives from drug-makers, etc. Over the past decades many target levels have been changed (eg. blood pressure, cholesterol levels, PSA levels), dramatically increIDg the number classified as having a particular condition. (Welch adds that prostate cancer can be found at any PSA level – about 8% for those with a PSA level of 1 or less, over 30% for those with a level exceeding 4; most are benign.)
The second is that treating those with eg. severe hypertension benefits those patients much more than treating those with very mild hypertension or ‘prehypertension;’ the result is treating those with lesser ‘symptoms’ can easily cause new problems that outweigh the value of the hypertension treatment.
The third is that Welch believes it is usually more important to treat those with disease symptoms (eg. pain) than those without.
I read Overdiagnosed this morning, and I strongly urge you to read it, too. If you’ve ever wondered why our country’s healthcare costs are skyrocketing even though our health outcomes lag behind the rest of the industrialized world, this book has the answers. We are overtested, overdiagnosed and overtreated. But sadly and paradoxically, this intensive use of "preventive" medicine has not improved our physical health or sense of well being — it has diminished it.
Dr. Welch builds a strong case that Americans are overdiagnosed in a clear, concise and compelling way. He provides anecdotal accounts of people who were seriously harmed by the overzealous use of modern, high tech testing. And he backs these stories up with findings from landmark medical research studies. As we move from chapter to chapter and disease to disease, we see the same patterns emerge: thresholds for "illness" are lowered and suddenly tens of millions of people are diagnosed and treated for mild or nonexistent "diseases" that never would have harmed them.
Dr. Welch identifies the key players who brought American medicine to this sorry state — big pharma and medical products manufacturers hungry to increase profits, doctors who order unnecessary tests to avoid malpractice lawsuits, and overzealous patient advocacy groups who press for action in the absence of any scientific evidence of improved outcomes.
Dr. Welch explains key concepts like "lag time bias" and "overtreatment bias" that enable you to see why the benefits of aggressive preventive medicine are far less than you have been led to believe. Once you understand terms like these, you will never again be swayed by misleading advertising or public health campaigns.

I’m in the Air Force and my job required me to pass the SY0-401. Many people failed with the book that was given to us, but this book helped me pass with an 806. Good luck to all who takes the test!
Another great study guide by Darril Gibson. Passed the Sy0-401 with just this book and 2 weeks of time. The test has some questions that i know i got wrong but i disagree with the way they worded it. I finished the test in about an hour. My advice is to read the question once, choose your answer, and all the possible choices followed by the question again. I got tripped up a couple of times on the wording, of what exactly they wanted and only after rereading the possible answer choices did I realize their request. They also have a few simulation questions without going into greater detail some of these were great others were a headshaking why.
Darril also has a twitter feed that you can follow for updates and more information around study material. Even if you’re not taking the test and want to learn more about Security, for under $10.00 its really a great cost for what your learning and having all organized in a way that will permit an east pass on the test.
Mr. Gibson has hit another home run with this installment of his CompTIA Security+ Get Certified Get Ahead: SY0-401 Study Guide. Covering all of the new material added to the latest set of objectives, this guide is well written and logically organized. Explanations of key concepts are clearly articulated and insightful. The practice questions are properly created and very useful for preparing for the exam. I especially love the Remember This tips found throughout the guide. In fact, students of Security+ would be wise to memorize each one of them.
As an active CompTIA Certified Technical Trainer who has trained hundreds of students on Security+, I recommend this guide to my students each week without hesitation. Too bad I can’t just teach directly from Mr. Gibson’s materials for my Security+ and Network+ courses. His study materials are excellent.

The Hunger Games (Trilogy) is one of the most "unputdownable" books to enter the teen market in a long time. The cliffhangers at the end of each volume are so intense, you can’t help but continue on. Knowing this in advance, I decided against reading the series last summer despite the fact that everyone was talking about it. I waited the extra year, and I’m glad I did–even a week was torture when it came to getting my grubby mitts on a copy of Mockingjay.
For the record, this isn’t a series for everyone. You will be drained emotionally by its end. The Hunger Games is one of the grimmest dystopian worlds I’ve encountered in literature. A lot of characters die, and their deaths aren’t pleasant. This series may not be for you. Then again, those who know me well would say it’s not for me, either. I’m one of the most squeamish people you’ll meet, and The Hunger Games more closesly resembles the movie Battle Royale than I thought it would when I started reading. I really enjoyed the series, though. There are scenes so poignant, they’ll stick with me. Between this and Stieg Larsson’s Millennium Trilogy, I’ve found that even squeamish ole me can still enjoy a disturbing book if it’s thought-provoking and well-written.
Now that I’ve warned you about the contents, let’s move on to the meat of this review. It’s hard to go in-depth without giving a lot away, so I decided to focus on the trilogy as a whole instead of singling out Mockingjay and reviewing it on its own (though I do have a paragraph dedicated to it further down). A brief synopsis for the uninitiated:
The trilogy takes place in the future. The USA has been destroyed; in its place is Panem, which consists of thirteen districts and a Capitol city.
First, as The Hunger Games are wildly popular at this time, I really did not want to like them as much as I did. There’s a snobbery in me that tends to assume that extremely popular fiction has merely reached a low common denominator for the unwashed masses, and them alone (see Nicholas Sparks and Stephanie Meyer). That said, contrary to my expectations, I did not merely enjoy The Hunger Games. I was devastated by them.
I devoured the whole trilogy in about a week and a half. I’ve no doubt it wouldn’t have taken that long if (a) I didn’t have a day job, and (b) I’d had access to all three books in one sitting. If this seems extreme, well, it is. Not afraid to say it.
The framework is a common enough idea in literature: it’s fiction of dystopia (there are hints that the country of Panem has replaced the now-dead USA) with an oppressive government and pockets of oppressed citizens. Leaders rise, conflict ensues, and the fates of citizens and nation alike are challenged, burned, and reformed (almost never in neat-and-tidy ways, either).
This isn’t to say that the plot is formulaic, because the specifics of the Hunger Games themselves as modern gladiator-style melees to the death is unique and compelling in the future context. Katniss Everdeen as the first-person protagonist does not fit into any formal compartments either, and her inner conflict, selfishness, selflessness, good decisions and bad ones all present emotions that not only compel and frustrate, but also ring true.
While the storytelling is clean and the events easily maintain interest, it’s the crushing human emotion and condition that propelled me through and left me an emotional wreck at the end of each novel.
